Disability Determination Assistance
Disability Determination Assistance services assists consumers with disabilities who have experienced difficulty in obtaining Supplemental Security Disability Income (SSDI) or Supplemental Security Income (SSI) awards. To be considered for disability benefits an individuals, Between the ages of 18-65, must have a disability that prevents them from working for a minimum of 12 months.
Independent Living Specialists trained in disability benefits eligibility assist eligible individuals with the disability determination process. Once disability benefits are obtained, the staff will link individuals with vocational rehabilitation sources to facilitate their return to employment and self sufficiency. Eligible individuals may be referred by a counselor, caseworker or other support professional.
LEAP staff will assist individuals in applying for or appealing denials for Social Security Disability Benefits, and securing SSI or SSDI, and/or medical coverage as an initial step towards self sufficiency by providing:
- Assistance with obtaining medical records
- Assistance with filling out medical or other necessary forms
- Assistance in obtaining necessary supports such as housing, child care and food stamps
