As a provider for Ohio’s HOME Choice Program, LEAP’s Nursing Home Transition program assists people in Cuyahoga, Erie, Geauga, Lake, Lorain and Medina counties who wish to move from long-term-care facilities into the community.
Independent Living Specialists help individuals identify their housing needs and preferences, find a suitable home, and purchase necessary home furnishings. In addition, they identify appropriate community resources and provide support after a move through telephone calls and in-home visits. Such support is invaluable as individuals readjust to living independently. LEAP also offers highly personalized training in skills needed to live independently, such as budgeting and cooking, which may be helpful to those rejoining community living.
