LEAP Empowers 1,288 to Live Independently in 2024

In the 2024 program year, LEAP made a significant impact by engaging with 1,288 individuals with disabilities and the aging population, supporting their decision to live independently within their community.
Benefits Assistance
- Assisted 398 people with navigating their benefits.
- Performed 376 benefit screenings.
- Submitted 71 benefit applications.
- Completed 13 benefit analyses.
Meaningful Employment
- Supported 112 people in securing meaningful employment.
- Provided Site Development/Site Coordination for 14 consumers.
- Performed employment assessments and career explorations/discoveries for 19 consumers.
- Gave job retention support to 19 consumers.
- Encouraged 24 young adults to explore different roles during our Summer Youth Work Experience (SYWE).
- Bridge, Employment, and on-the-job support, such as job coaching, were possible for 27 consumers.
- Job preparation services for 31 consumers.
- Supported 65 with obtaining job development/placements.
Recreational Opportunities
- Quantum LEAP had 307 participants who enjoyed low-cost adaptive recreation opportunities.
- Consumers had 34 different activities to pick from, with 68% participating in multiple activities.
Independent Living
- Our Independent Living team supported 405 consumers in reaching their independent living goals.
- Conducted HOME Choice eligibility assessments for 116 consumers.
- Assisted with 166 individuals' nursing home transitions, successfully transitioning 55 people out of nursing homes back into the community with the resources they need to live independently.
- Provided a range of support services for 180 consumers, ensuring they had the resources and support they needed to live independently.
