Living independently requires knowledge, planning, and support. LEAP’s services helped ensure that individuals understand their options for home- and community-based services and support and are ready for the journey towards independent living.

    • Individuals participated in LEAP’s programs and benefited from independent living skills training. These meetings provided assistance assessing and developing the skills needed to live independently.
    • LEAP is a partner for Ohio’s HOME Choice Program which transitions eligible Ohioans from a long-term care facility (e.g., nursing facility, hospital, or ICF-IID) to a home and community-based setting, where they received services and supports at home and in their communities.
    • Facilitated the transition of youth with disabilities to a future of independence and success.
    • Peer Support provided education and assistance to individuals with disabilities by helping them make informed and independent choices, setting and achieving personal goals, addressing multiple barriers, identifying options and creating solutions.
